Forms
Medication Administration
All medications must be kept in the nurse's office. A medication permission form must be completed by a parent or guardian. Students are not allowed to carry medications except for emergency medicines with written permission from a physician. The only exception is insulin and diabetic supplies.
Asthma
Students with a disgnosis of asthma are allowed to carry their inhalers at school with written permission from their primary health care provider. Permission forms to carry and self administer inhalers must be kept on file in the nurse's office. We strongly encourage you to have your child's primary care physician complete an asthma action plan to keep on file as well.
Seizures
Students with seizures should have a seizure action plan completed by their health care provider on file in the nurse's office. Emergency seizure medications must be kept in the nurse's office.
Severe Allergies
Students with severe allergies should have an allergy action plan on file in the nurse's office. Students are allowed to carry and self administer emergency epinephrine injections with written physician's permission. Allergy action plans and permission forms must be on file in the nurse's office.
Diabetes Care
Students with a diagnosis of diabetes may manage their diabetes during the school day with physician's permission. If a student needs assistance or supervision with their diabetes management during the school day please have their physician complete a diabetes management and treatment plan and return along with needed supplies to the school nurse. Feel free to contact or set up a meeting with the nurse for any questions or concerns.
